Home health care is an outstanding, cost-effective substitute to hospital and other institutional care. Service are provided based on the individual’s needs as defined by his or her doctor’s plan of care or their personal needs. This may include visits from skilled nurses, therapists, social workers and/or home health aides.
I AM Care is capable of handling requests for in-home care services as soon as you or your family need it. In order to ensure the suitable specialist is available, we recommended that at least 24 hours of notice is given.
There is information available about home health care services from a variety of sources, including hospital discharge departments, doctors, public health departments, or a quick online search for terms like Home Care, Home Health Care, or Nursing through a Web site search engines.
In most cases, home health care can allow a person to be discharged earlier from a hospital, reduces hospital admissions, and helps with a faster recovery in the comfort of their home. Most importantly, people are usually happier are more comfortable and happier at home and receive more care and attention from family members, friends, and home health care professionals.
I AM Care’s home health care services can be used on a short-term or long-term basis. It all comes down to your needs. Many of our patients use our services for a very short time, whereas others have been with us for very long time. The good news is that you will have a case manager who will keep in touch with you and your family to continually assess your situation and needs throughout your care.
